T-Shirt Order Form- Teams will not be guaranteed shirts UNLESS a form is received.

February 23rd- Mandatory Managers Meeting @ Diplomat West

• COPY of Roster
• COPIES of Birth Certificates
• Waivers- EVERY coach and player needs to sign a form
• Scheduling Grid-filled out with home dates and No Play dates
• COPY of Insurance Form

  All forms can be sent in sooner then the date they are due. Send check and all forms to P.O. Box.
